Jacqueline Diane Levene between the 29 October 2003 and 5 January 2004, failed to respond promptly to requests from the Bar Council for comments or information in respect of the complaint from Messrs Healys in that she failed to respond promptly to requests from the Bar Council dated 29 October 2003, 27 November 2003 and 18 December 2003. Jacqueline Diane Levene between 5 March 2004 and 7 May 2004 failed to respond promptly to requests from the Bar Council for comments or information in respect of the complaint raised of the Bar Council?s own motion dated 5 March 2004 in that she failed to respond promptly to requests of the Bar Council dated 5 March 2004, 31 March 2004 and 23 April 2004. Jacqueline Diane Levene failed to supply immediately upon being requested to do so such information as the Bar Mutual Indemnity Fund Limited required pursuant to its Rules, namely a Renewal of Cover Form for the policy year commencing 1 April 2003. Jacqueline Diane Levene failed to pay immediately when due the appropriate insurance premium for the policy year commencing 1 April 2003 required by the Bar Mutual Indemnity Fund Limited for the purpose of insurance against claims for professional negligence. Jacqueline Diane Levene between the 21 November 2003 and 30 January 2004, failed to respond promptly to requests from the Bar Council for comments or information in respect of the complaint from the Bar Council dated 21 November 2003 in that she failed to respond promptly to requests from the Bar Council dated 21 November 2003, 18 December 2003 and 15 January 2004. Jacqueline Diane Levene engaged in conduct which was discreditable to a barrister in that she continued to practise after 26 February 2004, contrary to the sentence of the Disciplinary Tribunal made on 10 March 2003 that she be suspended from practice as a barrister for 2 years from the date of pronouncement of sentence, that is to say 2 years from the 26 February 2004, and in particular acted in a trial at Southampton Crown Court between 8 and 11 March 2004.
